The total amount allocated for redistribution is 35,000 EUR. One applicant can receive a contribution for a rehabilitation stay at ADELI Medical Centre up to a maximum of. 2,000 EUR.
The contribution can only be used to fund the treatment program, therapies, and procedures related to the rehabilitation program. It does not apply to payments for services related to accommodation, such as lodging, travel, meals, etc.
How to proceed:
On the website www.nadaciaspp.sk, in the Grant Programs section, click on the Area Support for Disadvantaged and then on the program „One step forward“. There you will find it application form. Download it, fill it out, print it, and sign it. The application for financial contribution must be sent by mail to the address SPP Foundation, P.O. BOX 147, 810 00 Bratislava by November 23, 2016 at the latest. If you have not yet undergone rehabilitation stay at Adeli Centre, attach a medical report from a neurologist no older than six months to the form. If you have already undergone the stay, you do not need to attach a medical report.
Your request will be registered and assessed by the Selection Committee. The Selection Committee is composed of representatives from the SPP Foundation and representatives from the Adeli Foundation. The applicant's social situation will also be taken into consideration during the assessment. The list of selected applicants is approved by the administrator of the SPP Foundation. The assessment of submitted applications by the Selection Committee takes approximately 3 weeks. The applicant will be informed in writing about the provision/non-provision of the grant.
Rehabilitation must be undergone From January 1, 2017 to July 31, 2017. Applicants will be informed in writing of the Selection Committee's decision no later than December 16, 2016.
